Hmmm.... Judging from her uniform, I'd say she's from Lucky Star, but I could be wrong.

I don't recognize her, so I don't think it's a Lucky Star character, but I barely remember most of it but for all I know, she could be.
But she could just as easily be from a visual novel, which is likely if it's from Japan itself. And there's more VNs than anime, so the possibilities are endless.

The teddy bear is a distinctive feature, but i have no idea either.

Sango Himeyuri from To Heart 2. She has a younger twin sister named Ruri. Sango sometimes has a teddy bear.
