For me, it's a three way tie between Ladd Russo, Graham Spector and The Rail Chaser from Baccano.

You just have to love how crazy Graham is:

Ladd is also a psychotic killer and the Rail Chaser....well, he kills people by putting their faces on rail tracks while on a moving train.

Well I'd probably have to go with Fraken Stein from soul eater! He's gotta dissect everything! And after the madness starts getting to him his face is drawn in such a creepy way it makes my skin crawl xD. Not really all that insane just creepy.

Haruko (FLCL) That chick is DELIGHTFULLY insane. She has to be in the top five "Most Insane Characters" for sure.

Excel (Excel Saga) Granted, nobody on that show is normal, but she's got to be the craziest.

Nino (Arakawa Under the Bridge) She is definitely "an OVA short of a series". She has money but she chooses to live under a bridge. She has a comfy bed but she chooses to sleep in a DRAWER UNDER THE BED. Lol, what's up with that!?

T.K. (Angel Beats) I've only seen him for about five seconds but HE'S CRAZY!!

Dokuro-chan (Bokusatsu Tenshi) Yeah..........you want to stay far far away from her.....rofl. PIPIRUPIRUPIPIRUPIIIIIIIIIIIII!!

Shion (Higurashi No Naku Koro Ni) Holy..............crap. She scares me. Like "I want my mommy"-type of scaring.
