I would have to say when the sailor scouts all dissapeared in the 2nd last episode and Usagi was crying it was so sad, also when Usagi for sure knew Mamrou was dead.

Lt.Hues from Full Metal Aalchemist:This had to of been the most unexpected death in anything I've have ever watched.Right before this episode came on,I had said to myself that he was my favorite character in FMA and then he died,I was pretty damn shocked.
Julia from Cowboy Bebop:Because Spike tried so hard to find her and then she just ends up getting killed.
Jims crush in OutlawStar:I can't remember her name since she was only in one episode But anyway.Jim ends up meeting this girl and he grows a little crush on her.They meet up the next day but this time their little meeting was cut short,cause they both had business to handle.Jim gets back to his space ship and says something like"lets handle this quick,I have something I have to do later"with a big smile on his face.They end up haveing a space battle with another ship,which happens to be jims crush.Jim had no idea that that was her and vise-versa, so Jim and the other OutlawStar crew members end up killing the girl.Jim gets all happy because he was supposed to meet up with her again at the same spot later that day.So he rushes back to that spot,but she was nowhere to be found ofcourse,since he killed her,but he had no clue.

Also, Aeris death was NOT sad, I felt NOTHING. Why? Because she was a useless character anyway. Why was she useless? because she died of course! Paradox?

Kawada from Battle Royale. The dude was a tank the entire time, but died at the very end. I liked kawada, one of my favorite students...His death was made of suck and gay >=(
The way Homura failed in Sayuki was kinda sad. All he wanted was a new world without pain. Sadly, he'll have to join his loved one in the afterlife(if he ever reached her).

Finally, someone who didn't care about the death of Aeris.When she died,I couldn't have cared less,I never used her anyway.
I totally forgot about Kawadas death in Battle Royale,that did suck but I think it made the overall ending better.

The saddest moments of death in anime would have to be./...
Genkai's death in yu yu hakusho and also Yusuke's second death because the first time he died you hardly knew the character.
