Missing lines in Sword Art Online's Yume Sekai's transliteration (and translation)

Between the 4th and 5th stanzas, there should be another stanza that was omitted.

Keeping with the formatting of the original, I've transliteration/translated those two lines:

Osanai RIBON wo suteta yoru kara mita yume wa
Kagayaita machi de risou no hibi wo okutteta

Ever since the night that I threw away the childish ribbon, the dream that I saw was
Me having spent ideal days in a town that sparkled.

Also, while I'm at it, the last word of the 4th stanza in the transliteration (hibiiteiku) should be "hibiiteku" (the same mistake is repeated at the end of the 3rd to last stanza), and the last word of the 5th stanza from the bottom in the transliteration (tsuzukeru) should be "tsuzuketeru".
